Prepare before you start

Read the eligibility page and the complete form first. Note which sections you complete, which require a professional or other authorised person, and what supporting material is requested. Requirements may differ for an online application and a printed form. Ask the program for an accessible format if the available form is difficult to use.

  • Check that the form belongs to the correct state or territory and is the version currently linked by the program.
  • Write down the application questions you need help understanding.
  • Read the photograph instructions before taking or printing photographs.
  • Identify the professional or service provider the form permits to verify information.
  • Check signatures, attachments and submission instructions together before sending.

Describe the support you actually need

A useful preparation exercise is to describe a typical community activity: arriving, moving around, communicating, using facilities and returning home. Think about where another person provides assistance and what would happen without that assistance. Share relevant examples with the professional completing the form so their evidence reflects your circumstances.

Answer honestly and follow the questions asked. Avoid copying someone else’s application, overstating support needs or assuming a diagnosis guarantees approval. If your circumstances are complex or you are applying for a child, ask the official program how its criteria apply before proceeding.

Submit through the official channel

Before sending personal information, check the destination against the official page. Keep a copy of what you submit and any reference number or acknowledgement. For a process with several contributors, confirm that every required section has been completed. The program may need more information before it can make a decision.

After a decision
