Start with the Victorian program

Victoria’s Companion Card program is managed by the Department of Families, Fairness and Housing. The official website is the place to check eligibility, obtain the current application and read the terms of use. If you live elsewhere, begin with the program for your home state or territory rather than submitting a Victorian form.

The program is for eligible people whose disability creates a lifelong need for a high level of attendant care when participating in community activities. Applying in Victoria

The official application page provides online and postal options. Online applications involve a section for the applicant, or someone assisting them, and a section for a health professional. Follow the instructions on the live official page, including the order of completion, photograph requirements and supporting information.

Before starting, make a note of the tasks for which you need support during community activities. Bring the current instructions to the professional who will help with the application. Using your card at a venue or event

Begin with the official Victorian affiliate information and confirm participation for your chosen activity. Ask the booking team how to arrange the companion ticket and any access seating. A venue’s participation does not tell you everything about the access route or facilities.

For a city outing, consider the distance between your transport stop, the entrance and the activity space. For a regional trip, allow time to confirm accessible stops, opening hours and the return journey. An event at a familiar venue may use a different entrance or seating layout, so use the details for the actual session you are attending.

Transport needs a separate check

The Victorian program links to public transport information, but planning a journey still involves the operator’s current accessibility and service advice. Check the services you intend to use, any planned disruptions and how to request assistance. Confirm ticketing with the transport operator rather than assuming an attraction’s booking rules apply to transport.

Renewals, replacement cards and changed details

Use the official Victorian website for renewal, replacement and update instructions. If you are waiting for a card and have a booking planned, ask both the program and the venue what you should do. Do not assume a receipt, old card photograph or application reference will be accepted in place of a current card.
