Using your card

Replace/Update cards

Lost or damaged cards

If you lose or damage your Companion Card you can request a new one to be sent to you by contacting the Companion Card team.

Changing your address or contact details

To update your address or contact details, such as telephone or your email address, contact the Companion Card team. You will not be sent a replacement card.

Change the name on your card

You may need to change the name on your Companion Card if you have recently been married or divorced, or if you have changed your name by Deed Poll. You can only change the name on your card if you can provide evidence that your name has been legally changed. You will need to provide a copy of supporting documentation (such as a marriage certificate) before a change can be made. Contact the Companion Card team to discuss these changes. A replacement card will be posted to you.

Update your photograph

If your appearance has changed significantly and you are no longer recognisable from the photograph on your card, you can update it by contacting the Companion Card team. You will need to provide two identical, high quality, passport-sized photographs before a replacement card can be sent to you.
